My child is in good health and has my permission to participate in Cool Creations Summer Camp Program. In the event of illness or injury, I hereby consent to whatever x-ray, examination, anesthetic, medical, dental or surgical diagnosis or treatment and hospital care from a licensed physician as deemed necessary for the safety and welfare of my child. It is understood that the resulting expenses will be the responsibility of the camper’s parent or guardian.

Due to lack of space, finished art pieces MUST be picked up 2 weeks after last day of camp. We are not responsible for items left after 30 days. Please remember to pick up your kid’s items!

PHOTO RELEASE: We may take photos of program participants and of art projects to promote Cool Creations on social media and the work it supports in our community. We will not disclose names of participants.

REFUND POLICY: CAMP BOOKING IS NON-REFUNDABLE. A camp credit may be given for another camp if cancelation is made at least a week before camp start date and IF there is availability.
